Nurse Practitioner for @Home Medical Weight Management

 

We are seeking entrepreneurially minded Nurse Practitioners to perform in-home face-to-face evaluations, annually. As we help you build a weight management practice of your own, you will also review monthly online progress reports completed by your patients to determine whether ongoing medical therapy for weight management is warranted.

 

Each Nurse Practitioner will see their patients with increased body weight in person once per year to perform a history and physical examination, review blood work, and review medications for changes. Each patient will submit online progress reports which are reviewed by the Nurse Practitioner monthly. Patients that continue to meet criteria for treatment will have their weight management prescriptions renewed by the Nurse Practitioner. Patients will also be seen formally by the Nurse Practitioner at 3 and 6 months through our telehealth portal to review blood work, and review medications for changes.

 

Nurse Practitioners will have their own provider-patient web portal that provides a list of patients and their medical records. Management of other medications that need a change in dosage due to calorie reduction and weight loss such as hypoglycemic medications will be performed by Nurse Practitioners. Nurse Practitioners will also write prescriptions for medical nutritional therapy, lifestyle behavior modification, and exercise based on recommendations from patient interactions with nutrition, behavioral, and exercise coaches.

 

Requirements for the Position:

  • Current license in the state of practice with a physician collaborative practice agreement.

  • Current state drug regulatory registration and federal DEA license.

  • Comfortable with in-home evaluations.

  • Current driver’s license with good driving record and reliable transportation to travel for home evaluations.

  • Proficient in computer and mobile device skills including but not limited to encrypted multilevel login, uploading documents, e-mailing and texting, and completing online forms.

  • Must be comfortable with telemedicine including video conferencing, accessing and reviewing documents from tracking devices, and accessing and documenting in electronic health records.

  • Comfortable with recommending various diets, including traditional low-fat, low-carbohydrate, vegan, vegetarian, and anti-inflammatory etc.

  • Management of medical recommendations for many clients with multiple medical diagnoses, varying degrees of physical ability and increased body weight.

  • Excellent working knowledge of evidence-based medical treatments for obesity-related medical and behavioral conditions.

  • Ability to make quick and accurate clinical assessments of obesity-related medical and nutritional problems.

  • Excellent written, verbal and remote communication skills via video chat or phone working with primary care medical providers using a consultative style.

  • Ability to conduct brief patient contacts with proficiency in a variety of telecommunication methods including but not limited to telephone, texting, and teleconferencing.

  • Very good knowledge of pharmacotherapy for obesity and related conditions.

  • Ability to assist in the design and implementation of clinical pathways and protocols for treatment of selected obesity-related nutritional disorders.

  • Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent and equitable manner.

  • Ability to effectively represent company’s interests in the community and maintain effective working relationships among coworkers and public, private and professional groups.

  • Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or governmental regulations.

 

Duties:

  • House calls for initial History & Physical Examination and Patient Environmental Review (Including home and neighborhood).

  • Monthly review and assessment of symptoms checklist for each patient to determine suitability for the continuation of medications, and diet and exercise plans.

  • Alerting administrative medical staff of any medical problem related to diet or weight loss identified during the initial visit or during weight loss treatment.
