The Process
...is easy especially if you already have your medical records handy!
1. Schedule an APPOINTMENT with us online or we can arrange to come to you for a visit (if appropriate).
a. HOME VISIT - If you have active prescriptions for medications or therapies for your qualifying condition from your regular physician that can be verified before your visit, we will perform a house call.
b. STANDARD ONLINE VISIT - We will help you obtain medical records from your regular physician. You will have to initiate the request and pay any additional fees.
c. DISCOUNTED ONLINE VISIT - If you have in your possession valid medical records (a visit note, a procedure note, or a letter from your doctor on their official stationery) that document a qualifying diagnosis.
d. STUDENT AND VETERAN ONLINE VISIT - If you have a valid student or military identification card you will be eligible for a further discount if you also have in your possession valid medical records (a visit note, a procedure note, or a letter from your doctor on their official stationery) that document a qualifying diagnosis.
2. Your ONLINE APPLICATION needs to be INITIATED by, you, THE PATIENT at https://mo-public.mycomplia.com/#!/signin. Only applications submitted through the Missouri Department of Health and Senior Services online registry will be accepted.
3. We fill out the MISSOURI STATE PHYSICIAN CERTIFICATION FORM after your visit and send it to the state with your required medical records by uploading it to the ONLINE APPLICATION that you’ve started.
4. Once the ONLINE APPLICATION to the State of Missouri is completed, the State will contact you to verify your identity, residency, and to perform a firearms check. You cannot receive a Missouri State Medical Marijuana Identification Card if you also hold a firearm permit. Again, only applications submitted through the Missouri Department of Health and Senior Services online registry will be accepted.
5. If all checks out, you are good to go and the State of Missouri will send you an Identification Card that will allow you to purchase Medical Marijuana (at a local dispensary) or grow marijuana for medicinal uses depending on your application type.
Note: This process is not like having a "prescription" in the traditional sense, in that your insurance company will not pay for or reimburse you for your Medical Marijuana products. Additionally, your regular physician may not be able to certify your Medical Marijuana Identification Card application due to certain insurance arrangements with the Federal government because Medical Marijuana is not federally legal.

As part of the Physician Certification Form, physicians will be required to attest that certain statements are true. These attestations provide a framework for the certification process and are the primary way in which the State of Missouri has conveyed the standard of care it expects medical marijuana patients will receive. These expectations can be summarized as follows:
-
In the case of non-emancipated individuals under the age of eighteen, physicians must receive the written consent of a custodial parent or legal guardian who will serve as a primary caregiver for the qualifying patient before certifying that patient.
-
The physician must meet with and establish a patient-physician relationship with the qualifying patient per Missouri state telemedicine regulations.
-
The physician must review the qualifying patient’s medical records or medical history and the qualifying patient’s current medications and allergies to medications.
-
The physician must have a conversation with the qualifying patient (or the qualifying patient’s custodial parent or legal guardian) about the patient’s current symptoms.
-
The physician must create a medical record for the qualifying patient regarding the meeting and maintain the qualifying patient’s medical record as required in 334.097, RSMo.
-
The physician must have a conversation with the qualifying patient (or the qualifying patient’s custodial parent or legal guardian) about the risks associated with medical marijuana, including any known contraindications applicable to the patient.
The physician must have a conversation with the qualifying patient (or the qualifying patient’s custodial parent or legal guardian) about the risks of medical marijuana use to fetuses and to breastfeeding infants, if applicable.
